OLYMPIA AREA:

PACT Case Manager of Record

This member of the PACT transdisciplinary team has training in one of the mental health professions (e.g. nursing, social work, rehabilitation counseling, or psychology) and clinical skill and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to program clients with severe and persistent mental illnesses under the clinical supervision of the team leader. Functions as a clinical member of the transdisciplinary team and provides treatment, rehabilitation, and support services.

QUALIFICATIONS:

·        Post-secondary school education and training is required, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illnesses.

·        Must be licensed or certified to work in the field.

·        Abide by a code of ethical and practice standards, and be committed to a process of lifetime knowledge and skills development.

·        People with master’s or doctoral degrees in nursing, social work, rehabilitation counseling; or psychology are eligible as are registered occupational therapists.

·        Must have a strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.

·        Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment are essential..

Supervisor - Triage/Crisis Stabilization & Transitional Unit

Provides direct supervision of program services provided by the Triage and Crisis Stabilization and Transitional Unit. Identifies and develops program opportunities to meet community and client needs and ensures efficient and cost effective program operations in full compliance with all applicable BHR, RSN, funding/payer, federal and state requirements.

QUALIFICATIONS:

·        Master’s Degree in psychology, counseling, or related field, with two years direct experience working with the seriously mentally ill. Must qualify as a Mental Health Professional per WAC 388-865-0150.

·        Recent management experience including, but not limited to, direct supervision of staff and accountability for service delivery. Possesses excellent leadership, team building, decision making and planning skills.

Garden's Housing Staff - On call

On-call position working up to 19 hours per week to provide on-site presence to monitor safety and the ability for all to have the peaceful enjoyment of the dwellings. To use de-escalation skills to encourage sound tenant skills and alert appropriate authorities to prevent crisis situations. May deliver face-to-face interaction with authorized clients diagnosed as having chronic and persistent mental illness, emotional and/or co-occurring disorders. This position does not have the purpose of provision of support to the ongoing stability of tenants. Rather, it is there for on-site monitoring of the physical plant and supporting the success of tenants.

QUALIFICATIONS:

·        BA degree in Psychology, Social Work or related field of study and/or one year experience working with populations having chronic mental illness, emotional disorders and chemical abuse/dependency issues.

·        Demonstrates initiative and success in providing client centered interaction, solution-based approaches to learning, motivation techniques and positive reinforcement skills.

ICM Case Worker - On call

On-Call position working up to 19 hours a week to Deliver face-to-face Assisted Support and Check-in Support to ICM authorized clients diagnosed as having chronic and persistent mental illness, emotional and/or co-occurring disorders. Complete daily assignments providing face-to-face support services to clients who reside in BHR housing, Adult Family Homes and/or in the community at large. Assisted and Check-in Support includes: medical appointments, medication monitoring, transportation, meaningful daily activities, personal care, housekeeping, in-home emotional support, socialization skills, symptom management and other related services.

QUALIFICATIONS:

·        BA degree in Psychology, Social Work or related field of study and/or two plus years experience working with populations having chronic mental illness, emotional disorders and chemical abuse/dependency issues.

Licensed Practical Nurse

Provides nursing care, medication administration and other duties in an acute care psychiatric setting.

QUALIFICATIONS:

·        LPN currently licensed in Washington State

·        Two years minimum working with acute or chronically mentally ill population OR two years working in Nursing Home setting, OR other residential nursing care facility where clients with mental illness or developmental disabilities reside.

·        Provide nursing care for patients, including, but not limited to: vital signs and pain assessment, wound and skin care, hygiene, diet and nutritional care, intake and output monitoring as prescribed, LOC assessment, musculoskeletal function, and monitoring patient response to medications and other treatments.

·        Document all findings and assessments in patient clinical record. Serve as medication nurse as assigned

·        Work with the clinical team to plan and implement relevant treatment for patient population.

·        Meet with Milieu Supervisor and Nurse Manager for supervision. Participate in E & T staff meetings.

MIO Coordinator

Responsible for providing assessment, intervention and referral for mentally ill persons who have been arrested or are nearing release from jail. Appropriate alternatives are recommended to the prosecutor, and the offender is enrolled in community-based programs for which he or she is eligible.

QUALIFICATIONS:

·        Mental Health Professional as defined in WAC. Masters Degree in a social services discipline or related field

·        Two years experience providing services to severely and persistently mentally ill adults or emotionally disturbed children Extensive experience in the assessment and treatment of serious and persistent mental illness Experience with mentally ill offenders desirable.

Mental Health Aide - On Call

Provides a basic level of general behavioral and psychiatric care as described below on a unit for mentally ill clients on a specific shift, participate in the overall treatment program, and other related work.

QUALIFICATIONS:

·        Possession of a valid Counselor's Registration or licensed Certified Nurse's Aide in Washington State.

·        A.A. degree in mental health/social services or 6 months experience working with persons with a chronic mental illness

·        Responsible for assisting the consumer in daily living skills.Knowledge of current first-aid methods and cardiopulmonary resuscitation.

·        Knowledge of working with assaultive clients and de-escalation techniques. Knowledge of effective therapeutic relationships with mentally or developmentally disabled clients.

·        Gives a basic level of general behavioral care to mentally or developmentally disabled voluntary, involuntary or judicially committed clients.

·        Provides assistance with preparation of meals, provision of clean lines and other activities of daily living.

·        Works with other disciplines as part of the treatment team to plan and carry out an overall treatment program for the clients in an assigned unit. Gives first aid as needed.

·        Responsible for documentation/paperwork concerning services provided, activities and behaviors of the consumer, consultations, crisis incidents, and coordination/plans that involve the case manager.

Senior Business Systems Analyst

The Business Systems Analyst will be responsible for working with the Thurston-Mason RSN, contractors, staff, and BHR's system analysts to define, document and implement business process and systems solutions that optimize end-user productivity and the overall client experience. This role requires the analyst to be knowledgeable of healthcare enterprise systems, data quality analysis and data management.

QUALIFICATIONS:

·        Bachelors degree with 4+ years of experience in a similar role within the healthcare industry.

·        2+ years of experience with a clinical system. CMHC/MIS experience is a plus.

·        Strong skills in business process analysis, data quality analysis, and system integration.

·        Experience with project documentation including business requirements, process flows, QA test plans and end user documentation.

·        Experience with software quality assurance, gathering requirements, delivering project scoping, and implementing enterprise solutions in the healthcare industry.

·        Experience with Systems Development Lifecycle and Project Management methodologies.

·        Knowledge of MHD/DSHS data and HIPPA requirements is required.

·        Proficiency with Microsoft Project, Excel, Word and Visio a plus.

MIO Transitional Case Manager

Provides screenings to mentally ill offenders in county and local jails to determine eligibility. Those offenders determined to be eligible are to receive a mental health assessment, assistance to expedite applications for new or re-instated Medicaid benefits, and to expedite, facilitate, and coordinate their return to the community.

QUALIFICATIONS:

·        Masters Degree in a social services discipline or related field

·        Mental Health Professional as defined in WAC.

·        Two years experience providing services to severely and persistently mentally ill adults

·        Experience with mentally ill offenders desirable

·        Screen inmates identified by jail staff as needing mental health services. Determine if client meets the eligibility criteria established by the RSN.

·        When the inmate meets eligibility criteria a complete mental health assessment will be performed to determine if the client will meet access to care standards.

·        Staff will expedite, facilitate, and coordinate the inmate's return to the community and access the necessary community resources to make that return a safe and successful transition back into the community.

CD Counselor Aid

On-Call position delivering limited direct counseling, evaluation and casework services to clients of BHR Recovery Services. Scheduling of hours will be negotiated with the supervisor and may include some evening work.

QUALIFICATIONS:

·        Have basic knowledge of the theory and practice of chemical dependency and substance abuse treatment. This knowledge may be gained through previous work experience or college level course work.

·        Relevant course work would include nine quarter or six semester credits from an accredited college or university, with a minimum of three quarter or two semester credits in each of the following course topic areas: a) Survey of chemical dependency b) Physiological actions of alcohol and other drugs c) Chemical dependency counseling techniques

·        Be registered as a counselor in the state of Washington. (Chemical Dependency Professional Trainee (CDPT) status is a requirement of this position).

·        Understand and adhere to state and federal confidentiality regulations and sign an oath of permanent confidentiality covering all patient information.

·        AA degree required. BA degree preferred

Psychiatric Nurse Practitioner

Utilize the nursing process by collecting data; making assessments, planning, implementing and evaluating treatment to promote optimal mental health for consumers. This position will be for a new adult supervised facility located in Tumwater, WA.

QUALIFICATIONS:

·         As defined by WAC 275-56-175, a Psychiatric Nurse Practitioner must be an Advanced Registered Nurse Practitioner (ARNP) as currently licensed in Washington State with prescriptive authority.

·         Current DEA license and necessary certificates/working agreements for prescribing Schedule II-V drugs.

·         Minimum of two years experience in the treatment of the mentally ill.
